Practis is excited to announce the launch of Secure Online Vanderbilt ADHD Assessment Forms. The NICHQ Vanderbilt Parent and Teacher Assessment Forms are a diagnostic tool that are completed by parents and teachers for use in determining whether a child has ADHD. Provided as software as a service, our new on-line forms provide an easy and secure alternative for health care providers to easily collect this needed assessment data on-line.

In addition to our online assessment forms, the Vanderbilt ADHD Assessment Form Package includes the American Academy of Pediatrics ADHD Toolkit.
